NEW YORK (Reuters Health), Nov 17 - Carotid plaque surface irregularity, even after adjusting for degree of carotid stenosis and plaque thickness, is a strong, independent predictor of ischemic stroke, researchers report in the November issue of Stroke.
"Narrowing of the carotid arteries is known to increase the risk of stroke," senior investigator Dr. Robert L. Sacco noted in comments to Reuters Health. "In this study, we also show that certain carotid plaques may be more risky than others."

Among a multiethnic group of 1,939 stroke-free subjects having a mean age of 69 years, high-resolution B-mode ultrasound revealed carotid plaque in 56.3%. Irregular plaque was seen in 5.5% of subjects, Dr. Sacco and colleagues at Columbia University, New York, report.

Over an average follow up of 6.2 years, 69 patients experienced ischemic stroke. According to the researchers, in comparison with no plaque, the presence of carotid plaque with an irregular surface was independently associated with increased risk of ischemic stroke. The adjusted hazard ratio was 3.1.

The cumulative five-year risk of ischemic stroke in patients with an irregular plaque surface was 8.5% compared with 3.0% in subjects with regular plaque and 1.3% in those with no plaque.

"Irregular or ulcerated plaque had the greatest risk of stroke even after adjusting for multiple vascular risk factors," Dr. Sacco concluded. "Screening tests such as a carotid ultrasound," he added, "may be helpful in identifying certain persons at high risk of stroke."
